We believe that the approach of our experienced psychiatric nurses who provide home health care is unique based on their specialized training and years of experience.  They are prepared and well-qualified to be an instrumental support to physicians and mental health professionals helping clients in the community achieve a higher level of wellness and stability to reach a shared goal of reduced hospitalization.

Our team of professionals includes the following individuals who are available to evaluate individuals in their homes to determine how a care plan including the home health team may improve outcomes for individuals challenged by behavioral health issues.  Clients who receive care by these nurses benefit greatly from a plan of care that is rooted in client advocacy and tailored to their individual needs and optimal level of independence.

KC: RN-C has been actively involved with service to psychiatric and dually diagnosed clients for over nineteen years, 13 of those years being spent with home care clients.  She also had a professional nursing background which includes years in settings such as intensive care units, inpatient psychiatric/substance abuse units and geriatric care programs.  In addition to her nursing education, KC has continued with coursework at Southern Connecticut State University with a focus on psychology studies.  Her work with individual clients, and their caregivers, is grounded in collaboration with the mental health practitioners and community resources to counsel and build a network of appropriate support around a client in need.  KC has maintained a special interest in mentoring and teaching colleagues to promote delivery of the highest quality in behavioral health care. 

LN: RN, BS specializes as a psychiatric nurse and now has almost thirty years experience in various psychiatric health care settings. LN holds an Associates degree in Nursing and a Bachelor of Science degree in Health Care Management and Community Mental Health.
Her first years in psychiatric nursing practice were spent in psychiatric hospital settings where she held supervisory and direct patient care roles including charge nurse on a neuropsychiatric evaluation unit which also accepted patients in need of detoxification.  This unit later transitioned to a dual diagnosis adolescent treatment setting of which LN was an instrumental participant in the development and delivery of protocols for assessment and treatment.  Upon leaving the hospital setting, she played a key role in developing the first clinical services for Harbor Health Services, a partial hospital program.  As the program grew, there was evolution toward an Intensive Outpatient Program and LN served as the clinician for the Dialectical Behavior Therapy tract.  LN has worked in home health care since 1989 as a supervisor of psychiatric home care and nurse case manager focusing on adults with a variety of psychiatric diagnoses and often concurrent medical issues.

AW: RN received her Associate of Science degree in Nursing from Bridgeport University and a Bachelor of Science degree in Special Education and minor in Psychology.  AW has worked with psychiatric clients since 1978; initially as a psych tech and then as a Registered Nurse.  She has worked in a variety of settings and with a variety of populations affected by disorders.  AW’s interest lies in working with individuals challenged by chronic mental illness and cognitive deficits with a focus on maximizing their independence and wellness in the community.  Her patient combination of therapeutic communication and counseling, together with a daily living skills teaching emphasis to foster independent function, has been highly effective.  AW’s approach is reflected best in her own words, “I try to listen to the client’s language and meet them where they are.”

CO: has been a RN for twenty-three years.  She attended Quinnipiac University and graduated in 1987 with an Associate’s degree.  She first began her journey as a nurse working as a medical nurse at Yale New Haven Hospital as a Cardiac Pulmonary nurse in the cardiac unit.  From there she also gained experience in Home IV Therapy and helped in Lyme disease studies at Yale and Danbury Hospital.  She left the hospital and worked in an assisted living facility located in Branford called Gardenside Terrace where she gained her knowledge with the geriatric population.  She has expanded her career since then and has begun working with both the medical and behavioral population in homecare for twenty years.  She has helped clients get the services they need with the Agency on Aging, Bridges and the PCA waiver.  These programs are to access the needs of the clients with their independent activities of daily living.  She is very thankful for being a nurse because it has given her the opportunity to gain experience in working in a diverse community.
