A Medical Social Worker (MSW) provides social work services to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/ Nursing Supervisor. Services are furnished by a qualified social worker with a license or by a qualified social work assistant with a license under the supervision of a qualified social worker. Some of the responsibilities include:
Schedules patient visits as necessary according to patients needs and as physician orders
Assists physician and other team members in understanding the significant social and emotional factors related to health problems
Comprehensive evaluation of psychosocial status as related to the patient’s illness and environment
Works with the family. Provides supportive counseling and assistance with adjustment to illness and lifestyle changes.
