Master Social Worker

A Master Social Worker (MSW) provides social work services to patients on an intermittent basis in their place of residence.  This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/ Nursing Supervisor.  Some of the responsibilities include:

  • Schedules patient visits as necessary according to patient/caregiver needs and as physician orders
  • Assists physician and other care team members in understanding the significant social and emotional factors related to health problems
  • Comprehensive evaluation of psychosocial status as related to the patient’s illness and environment
  • Works with the family.  Provides supportive counseling and assists with adjusting to health care needs and lifestyle changes
